Insurance renewals require evidence of CPD (continual professional development) in the last 12 months. Please forward photocopies of your certificates as evidence of CPD and of any additional fitness qualifications that you wish to be added to your insurance policy. You do not need to send copies of qualifications if they are already held by Fitness Wales.
Membership and Insurance are on an annual basis, with policies dated from the 1st of the month. Insurance is available to Members only. Insurance is for individuals not for businesses, and is for UK residents (ie. have a UK home address). Premiums include a small administration fee and are correct at time of printing, but may be subject to increase.
To process your application your details will be entered onto a database. Your details will not be passed to 3rd parties without your consent.
Fees are non-refundable.
There are no current incidents/circumstances that might give rise to a claim and I agree to abide by the Code of Ethical Practice which is available on application.
